Referral Process

We partner with case managers, social workers, and agencies across Maryland and the DMV to place clients in safe, structured housing. All referrals are reviewed within 24–48 hours.

Step by Step

How the Referral Process Works

From submission to move-in, we make the process as smooth as possible for case managers and their clients.

Submit a Referral

Complete the referral form online or contact us directly at (301) 887-3597. Include client background, funding source, and preferred room type.

24–48 Hour Review

Our team reviews every referral within 24–48 hours and contacts the case manager to discuss placement options and availability.

Intake Interview

We schedule a brief intake interview with the client to review house rules, expectations, and confirm the placement is a good fit.

Documents & Funding

Required documents are collected and funding authorization is confirmed before a room is assigned.

Room Assignment

A room is assigned based on availability, client preference, and funding source. Move-in date is set in coordination with the case manager.

Move-In & Orientation

On move-in day, the resident receives a full orientation, signs the lease agreement, and receives their keys.

Who Qualifies

Eligibility Guidelines

  • Adult (18+) in need of transitional or supportive housing
  • Referred by a case manager, social worker, or agency
  • Able to comply with house rules and structured living environment
  • Has an identified funding source (agency voucher, SSI/SSDI, GA, employment, or self-pay)
  • No active violent criminal history that poses a risk to other residents

Before Move-In

Required Documents

Please have the following documents ready before or at the time of move-in.

Identity & Benefits

  • Government-issued photo ID (driver's license, state ID, or passport)
  • Social Security card or proof of SSN
  • Proof of income or benefits (SSI/SSDI award letter, pay stub, GA approval)

Funding & Authorization

  • Agency voucher or funding authorization letter (if applicable)
  • Case manager contact information and agency name
  • First month's rent or deposit (per funding arrangement)

Health & Background

  • TB test results (required within 30 days of move-in)
  • Any relevant medical or behavioral health documentation (optional but helpful)
